1. Your name
2. Up to 5 poems, OR up to 5000 words of fiction or creative nonfiction, OR up to 5 visual art submissions (paintings, photographs, digital art pieces, etc.)
3. A biographical statement
4. An artist’s statement about the piece(s) submitted.
5. Your Venmo handle for payment
